General Category
a) Birth Certificate issued by the Municipal Corporation.
b) Proof of residence (Passport/Registered Lease Deed/Electricity Bill/MTNL Bill/Electoral Card).
c)One set of Photograph each of mother and father.
d)Two photographs of the child.
e)Fitness Certificate.
E.W.S Category:
a) Birth Certificate issued by the Municipal Corporation.
b) Proof of residence (Passport/Registered Lease Deed/Electricity Bill/MTNL Bill/Electoral Card).
c)Income Certificate issued by Revenue Officer.
d)One set of Photograph each of mother and father.
d)Two photographs of the child (at the time of admission).
e)Fitness Certificate.
